A new nonprofit is making waves in the Coachella Valley with a mission rooted in compassion, dignity, and cultural sensitivity. Triangle Care Management (TCM), launched in October 2025, offers free Care Management Navigation for aging LGBTQ+ adults and LGBTQ+ individuals with disabilities—connecting them to affirming providers, services, and resources tailored to their unique needs.
Founded to address the isolation and healthcare disparities often faced by LGBTQ+ seniors, TCM provides one-on-one support through dedicated Care Manager Navigators. These professionals assess each client’s needs in a welcoming environment and guide them through the complexities of aging and health care systems.
“The rapidly growing LGBTQ+ community in the Coachella Valley requires life management services that often seem unaffordable or difficult to locate,” said Kelly Watson, President of Triangle Care Management. “We provide a personalized path for each client that includes detailed information and referrals to LGBTQ+ affirming services.”
TCM’s services include referrals to legal and financial resources, socialization programs like Lesbians Embracing Aging, and future plans for LGBTQ+ shared housing. The organization also honors the legacy of its founder, Richard Bass, through the Richard Bass Legacy Circle—an initiative to fund housing development through legacy giving.
